Form Dialog Box

Good Day.

First time posting here. Great information. I remember in WordPerfect there was a way to create a pop-up dialog box asking for information:

Customer Name:

Type of Project:

Order Number:

Then based on the input, it would use it to fill out the form. I create a statement of work on a regular basis for customers. It would be such a help to be able to do this.

Sorry.... :oops:

I am using Word 2010. But would actually consider using OpenOffice since I am running Ubuntu 11.x. But I guess to play nice with the other folks in my office, 2010 would be helpful.
